REQUIRED FORMS
COACHES & MANAGERS: Please submit a completed Liability Release for every player on your team.  You may print a blank form and distribute to each player, or direct players to this website so they can complete the Liability Release form online and print it at home.  EACH PLAYER MUST HAVE A COMPLETED & SIGNED FORM BEFORE THE FIRST TEAM PRACTICE.  Also, please complete and submit a Team Roster for your team[s]. 

If you have additions or changes throughout the year, please submit an updated roster and the required Liability Release[s].

FIELD RENTAL RATES
Full Field
April through September $85 per hour 
October through March $100 per hour

Half Field
April through September $50 per hour
October through March $60 per hour

CHECK PAYMENTS
Please make your checks payable to LAKELAND and write your Athletic Field account number on the memo line of your check. 

ONLINE PAYMENTS
We are able to accept online payments via Paypal using your debit card or checking account...no paypal account is required! 

If you would like to request an invoice for your account, please email schedule.lakeland@gmail.com and place "request invoice" in the subject line.  An email response will be sent as soon as possible.
